Dynamic Hospice and Palliative Care is a community based company that specializes in identifying, treating and educating those that find themselves with a life limiting diagnosis. Our Inter-Disciplinary Team is comprised of many health care professionals who meet regularly to insure that our clients receive exceptional care. Patient quality of life is always Dynamic Hospice’s primary focus and we insure that quality of life is maintained by working closely with our clients, family members and their caregivers. Hospice is about continuing education and teaching the community about the availability of our programs for those with a life limiting illness. Dynamic Hospice’s staff members are well trained health care professionals that concentrate on preparing patients and their family members to deal with different stages of the disease process and to properly address issues as they arise. The family is made aware that if things become too stressful, Respite Care, General Inpatient Care or Crisis Care are always available options; all of this being done while keeping your loved one out of the hospital.
Choosing hospice care for yourself or your loved one does not mean that you are “giving up” on life. Hospice care focuses on providing a caring staff and comfort measures to maximize the quality of life. Dynamic Hospice decreases the fear and anxiety of caring for patients facing end of life challenges by offering the availability of skilled nursing visits 24 hours a day. If you or your family members are not sure about the need for hospice, please contact us to schedule a visit in your place of residence, our office, or your doctor’s office. A personal visit from a Dynamic Hospice team member will assist patients and families to put hospice into perspective for their specific care needs.
Dynamic Hospice combines teamwork with the family and the community to optimize the quality of life for your loved ones who choose to stay in the comfort of their home. We are also able to provide our services in a skilled nursing home or assisted living facility.
