Crisis Care
The practice of offering 24-hour care is called crisis care (CC). Continuous home care is to be provided during periods of crisis to maintain the loved one at home. When medically appropriate, hospice team members work in shifts of up to 24 hours for varied periods to help keep patients at home through a difficult time, rather than be admitted to a hospital.
This intensive level of care is appropriate when the patient or family, is having a trying time managing the care, and includes the following:
We understand at end of life, this is when hospice is most needed. So, we want to be there with the patient at the time of transitioning. Dynamic has a dedicated CC team with RNs, LPNs, and Certified Nursing Assistants that have training in this level of care.
Chaplains and Social Workers are also available during this time to meet the holistic needs of the family. It is our goal to provide comfort, maintain dignity, and allow the family to be a part of this process as much as they desire.
